Endocardial-only catheter ablation with substantial improvement in ventricular fibrillation recurrences in a patient with Brugada syndrome
نویسندگان
چکیده
Introduction Brugada syndrome is known to be a life-threatening disease provoking lethal arrhythmias such as ventricular tachycardia/ventricular fibrillation (VF), but non–implantable cardioverter-defibrillator (ICD) treatment strategy remains a topic of discussion. Substrate-based catheter ablation of Brugada syndrome has been reported to be effective, but much is still unknown, for example, the relationship between local abnormal endocardial potentials and ablation results.

Brugada syndrome is associated with scar and endocardial involvement: Insights from high-density mapping with the Rhythmia™ mapping system.
The authors report the first catheter ablation of Brugada syndrome in the literature using the Rhythmia™ mapping system. Learning points include: (1) low voltage areas can be documented while mapping in some individuals, suggesting that Brugada syndrome may not be a pure ion channel disorder; (2) typical long fractionated potentials can also be identified in the endocardium, supporting the need...
متن کاملEpicardial radiofrequency catheter ablation of Brugada syndrome with electrical storm during ventricular fibrillation
RATIONALE Brugada syndrome (BrS) is characterized by ST segment elevation at the J point ≥2 mm in the right precordial electrocardiogram (ECG) leads, in the absence of structural heart disease, electrolyte disturbances, or ischemia. It is a well-described cause of sudden death in young patients, especially in the age of between 30 and 40 years old. Here, we reported an unusual case of electrica...
متن کاملFirst case of epicardial ablation to coexistent J waves in the inferior leads in a patient with clinical diagnosis of Brugada syndrome
Introduction It is well established that type 1 Brugada syndrome (BrS) is associated with ventricular fibrillation (VF). Recently, Morita et al suggested in an experimental model that radiofrequency catheter ablation applied to the right ventricular (RV) epicardium might be more effective than radiofrequency ablation applied to the endocardium in order to eliminate ventricular tachycardia in pa...
متن کاملPerformance of electrophysiologic study in an asymptomatic patient with type 2 intermittent Brugada syndrome: To do or not to do?
Background: Brugada syndrome (BrS) is an inherited channelopathy, which is associated with sudden cardiac death due to rapid polymorphic VT or VF. There is no definite consensus regarding the management of asymptomatic patients. Some experts advocate close follow-up; others propose the programmed stimulation for risk stratification. We aimed to evaluate the benefit of complete atrial and ventri...
متن کاملSuccessful radiofrequency catheter ablation for electrical storm of ventricular fibrillation in a patient with Brugada syndrome.
The case of a 41-year-old man with Brugada syndrome (BS) who suffered electrical storms (ES) of ventricular fibrillation (VF) is presented. Although intravenous infusion of isoproterenol (ISP) suppressed the VF occurrence, he consistently experienced recurrence of VF following discontinuation of ISP infusion. Quinidine and cilostazol were ineffective. An analysis of VF episodes on electrocardio...
